PHOTO & VIDEO RELEASE
Without expectation of my compensation or other renumeration, now or in the future, I hereby give my consent to FLOUNOU ARTISTRY and its representatives to take photographs and/or video footage of me for the purposes of documenting my semi-permanent makeup procedures.
You understand that these photographs and/or videos may be used for the following purposes:
(a) My personal clinical records
(b) Training purposes for FLOUNOU ARTISTRY staff
(c) Marketing and promotional materials for FLOUNOU ARTISTRY, including but not limited to website, social media, print ads, etc.
This consent is given in perpetuity, and does not require prior approval by me.
You reserve the right to request removal of any published photos and videos in which I am featured.
